Mutations arise as a consequence of (at least) two mechanisms:
1. The enzyme that copies the DNA before celldivision is not perfect. Every now and then the wrong nucleotide is incorporated in the newly synthesized DNA strand. If the mistake is not corrected, it can lead to an error in the daughter double-stranded DNA in the next cell
2. Certain chemicals and ionizing radiation can damage the nucleotides in an existing DNA strand. Again, usually these defects are correct by enzymes that can do 'nucleotide excision and repair', but if this doesn't happen timely or correctly, a mutation can occur.
