First language for an english medium is always going to be ENGLISH

Second language depends. Basically it is Hindi in India. But in certain states like Punjab , Tamil Nadu, Andhrapradesh, Bengal , Assam , etc. the second language can be hindi as well as Punjabi, tamil, telegu, Bengali, Assamese respectively.

Third language can be any random language. Like many schools opt for even foreign languages like German , Spanish , French , etc. Indian languages like Tamil, Telegu, sanskrit,etc. are always an option.

Choosing for second language and third language is up to you, but first language in english medium is always going to be english.
