Hello friend!

It's requested is correct.

For example:

It is requested to all of you to take off your shoes before entering the clinic.

It's request is incorrect because we can't use it independently. Like we can't say-

It's request to follow him.

That's the reason why the above sentence would be considered wrong.

So according to me the correct one is

"IT'S REQUESTED"
