In general, the system also has three levels such as Gram Panchayat it is otherwise called as village level), second one is Mandal Parishad or Panchayat Samiti  or Block Samiti, it is otherwise called as block level, and finally Zila Parishad  which is called district level).

Of course it was formalized in 1992 by the 73rd amendment to the Indian Constitution.
