In Japan Originally it was the Emperor who, after consulting advisers, appointed the Prime Minister.
It was during the Meiji Restoration that the office of Prime minister of Japan was established in the 1880s.
But since the constitution of 1947, the Emperor still appoints the the Prime Minister but only appoints that who is designated by the Diet (Kokkai).
So, here we can have two answers.
1. Ito Hirobumi (1885–88) during Meiji period.
2. Tetsu Katayama (1947 -1948) after the Constitution of 1947.
The current Prime Minister of Japan -
Shinzo Abe since December 2012 till the date of writing of this answer.
