That square piece is called ventilator.That is built for air to come in and out in case,if all the windows and doors are locked, resulting in the passage of air to move inside and outside of the room.If the room is full packed ( if ac is not installed) then it would cause suffocation in the room,so in that situation a hole is always there in the top for ventilation.It is situated at the top to remove all the hot air from the room( as hot air rises up).
