Holmes is solving the murder of Culverton Smith’s nephew: Victor Savage. Victor was poisoned by Culverton.

Culverton tries to kill Holmes too by sending a box that is designed to infect him with the same tropical disease. Due to lack of evidence, Holmes decides to trick the murderer into confessing by pretending that he is in mortal danger. As Holmes had expected, the arrogant Smith confesses to killing his nephew as well as his (attempted) murder of the detective himself.
