Tom ‘created’ a toothache for himself because he did not want to go to school.

“Creating” a toothache here means that he pretended to have a toothache. It was Monday morning and Mondays meant the beginning of the study week.Tom was always ‘miserable’ on Mondays and “wished he was sick”, so that he wouldn’t need to go to school. The toothache was part of his plan. One of his milk teeth was loose and was almost ready to fall out, and he used it as an excuse to miss school. He hoped that his aunt, Aunt Polly would believe him, sympathise with him and let him skip school.
