Porters are mean to carry heavy load/luggage both on the head and on shoulders. They use a towel rotated in round shape, keep on center of the head to place heavy load in order to balance the pressure over head.
They feel heaviness over the head, but the round piece of cloth helps them tolerating the pressure and balancing the weight.

Also, pressure and area are inversely proportional to each other. Hence, when the area on which the load is placed increases, the pressure decreases automatically. This makes it less tiring for the porter and he can easily carry the heavy load.
