There are no gold standard tests that can help to diagnose luteal phase deficiency. Measurements of progesterone levels are not helpful, because progesterone secretions are pulsatile, and the levels may vary up to sevenfold in a space of a few hours.146,147 Basal Body Temperature (BBT) charting and endometrial biopsy are unreliable.145 In its opinion paper, the ASRM states that luteal length of 11–13 days suggests normal luteal function, whereas a length of ≤ 8 days is considered evidence of abnormal luteal phase.145
Luteal phase deficiency treatment options include ovulation induction (for example, with Clomiphene Citrate (CC) or Clomid) or hCG or progesterone supplementation during the luteal phase.138 The Cochrane review of progesterone supplementation for treating threatened miscarriages concluded that ‘…the use of progesterone is effective in the treatment of threatened miscarriage…’ and is not harmful to the mother or foetus.148
