Why is the President regarded as the nominal head of the state?

Assuming you’re referring to the President of the United States, he or she is not the “nominal” head, he or she is the actual head of state. The head of state is responsible for such ceremonial things as sending and receiving ambassadors, signing legislation into law, commissioning members of the military, etc. All of those things are duties of POTUS
