If you look at the concept of India before 1800, you can say it was about geographic borders of a common culture. Himalayas were just borders of a culture. Any mountainous region or state was more of a buffer state with partly plains culture and partly hill culture. The region of today's Uttarakhand, however, was sacred as a region of the upper Ganges. So, there was always a cultural influence there from the plains.

In this respect, only the lower hills and plains in the south of Nepal and Bhutan could be considered as “India”.

But India was never a political entity then as it is today. There were different rulers in each of the regions. The Mughals called their Empire as Gurkani. Then emerged the name Hindustan. But again borders were in a flux. Regions were annexed and lost.

Nepal and Bhutan were also not united with fixed borders as a country until India got its identity too. Today's Nepal is a bit older (1768). British India did precede today's Bhutan though.

Today's India is largely shaped by the politics post 1900. Nepal and Bhutan were not part of it.
