heparin prevents clotting of blood in blood vessels. But if there is internal injury, platelets break to release a thermoplastic substance. It initiates clotting.

Similarly, If you have blood in a test tube with heparin added in it. It's not likely to clot. But if you have blood in a watch glass or something with heparin added, air will make it clot.
