Sangams are not organised. Sangam literally means joining into one or merging into one. Whereever rivers join that point is called Sangam. (Sangamam in Tamil) . For example the point at which the river Godhavari joins the sea is called Godavari Sangamam.
Allahabad is called triveni sangamam, because three rivers join there. Ganga and Yamuna converging into one you can visually see. There is a third river which is considered to be underground is also believed to be joining at that point. Geologists believe that there was a river existing which was called Saraswati which is non-existent now.
