Dear Sir,
Sub :- Request for transfer to – reg.
At the very outset, kindly excuse me for taking your precious time to read my request in your busy schedule.
As you are kindly aware sir, recently I had undergone .....................(treatment).
While I was discharging my duties in ................., I had felt uneasiness and giddiness and the local doctors have advised me to go to a better hospital where further facilities were available.
Accordingly, I had to rush to ........... and after investigation, I was shifted to ............, on emergency and I had undergone ..........treatment .
The Specialist Doctors, who had attended me, have given lots of dos and don’ts, to avoid further damage to the health, of which few of the important ones are being brought to your kind notice:
1. To take the medicines as prescribed in time
as per the dosage.
2. To have bland diet without much spicy, oily, saturated fat etc.
3. To avoid any anxiety, stress/ strain etc.
4. To avoid lifting of heavy objects etc
5. To avoid climbing to higher altitudes etc.
As you would kindly agree with me, the prevailing conditions in the site, don’t allow me to follow any of the above precautions suggested by the Specialists. As I am staying as a forced bachelor due to school going children’s educational problems, I am not able to get the diet conducive for my illness. Also, in site, due to work pressure, I could not avoid doing strenuous jobs and climbing up the higher altitudes.
My family is wholly dependent on me and my inability due to various external constraints, to cope up with the advice of the Doctors, had put my entire family in to lot of mental tension and every one had lost their peace of mind and incase of any untoward incident happening to me, no proper emergency medical facility is also available in the nearby vicinity. i therefore humbly request you to kindly arrange to transfer me to other location(?) and oblige, so that I can undertake regular medical checkups in our organisation .
I along with my entire family would be grateful to you forever for this gesture.
