Introduction

If you intend to change your signature, you can send/give a request letter to your bank for a change in your signature. Make it a point to write a request letter in a way which is precise, addressing directly to the appropriate authority/person.

Requisition Letter Format

Here is the format of sample letter:

Your Name
Your postal address as recorded in your bank
Your contact number. If you wish you can give your mobile number
Your email id: You can give your mail id if you wish but not necessary

The Chief Manager
Name of your bank
Branch Name
Address of your bank branch

Date: dd / mm / yyyy

Dear Sir/Madam,

Sub: Request for change of signature for my account no. ____ (write your SB account number in the blank space)

I hold an SB (Savings Bank) account with your branch and my account number is ________ (write your SB account number in the blank space). 

This is to inform you that I have changed my signature for the operation of my account.

Request you to honour all my banking transactions for my SB account number given above with immediate effect, only if signed with my signature as it appears in this letter and not my old signature. Also, I request you to treat the signature appearing on this letter as my specimen signature for your records. 

In case you need my specimen signature in your printed forms, I would visit your branch and comply upon on receipt of such a document. 

Meanwhile, kindly acknowledge receipt of this letter and confirm the specimen signature on this letter for all future transactions.
