Subject: Seeking leave for three days

This is to inform you that I would like to visit my ailing grandmother, who had suffered a stroke and is currently admitted in the Siliguri District Hospital. I would be grateful if you kindly grant me three days leave, that is 23rd, 24th and 25th September. I will resume school from 26th again.

I assure you that I will not allow myself to lag behind in my studies and will finish all the pending work. If you kindly grant me permission for the leave, I will be highly obliged.
