I most humble beg to state that I am a resident under your jurisdiction. Day by day our locality has become a breeding ground of mosquitoes due to unhygienic environment . As a a result dengue breaks out in our locality .

Most people of our locality are suffering in this disease and some people are already hospitalize.

I request you to give permission to organize a dengue awareness camp and other medical facilities in our area .

Therefore, I shall be highly obliged if you kindly grant my application,and take the necessary staps as early as possible
