There is an application form for broadband connection (actually one need to apply for a landline and broadband at the same time, because broadband is attached to a landline number, there are combo plans and hence you needn't pay for landline). You need to just fill it in and give it to your nearby exchange.
Another easy way out is to give an SMS to 54141 if your are using BSNL number, else to 9400054141
Format is
BB*stdcode (in place of STD code you need to enter to areas STD code for eg. BB*0471) they will call you back and come to your home and collect application form.
