Subject: Leave application for 5 days due to personal illness

This is in lieu of my absence from school for the next few days. I have extreme fever and am fatigued of the cough and cold I have. I have been advised by my family doctor to take complete rest for minimum 5 days. Thus, I wouldn't be able to attend classes from 04thJuly to 08thJuly. Please permit me leave for the same.

As there was a sudden information came to my parents that an emergency for the need for donation of blood for my dad's friend in chennai therefore I was in a emergency to quickly move to chennai to donateblood . So I request you to kindly grant me leave for the previous week's absence.
