Answer:
Respected sir,
It is to state with due honour and respect that yesterday I had met a small accident at home as I slipped from my stairs and it caused me a severe pull in leg muscles. My bones are intact but my leg muscles are in worst condition that I even unable to bend it. I had attached a copy of Medical Certificate with this application so that I can be given a leave of one month for proper cure and health. I shall be very thankful to you for your kind response.

Subject - Application for using the school lift as I have a fractured leg.
Dear Sir,
I am ABC, a student from class 10th B. I am writing this letter to you requesting your permission to use the school lift as I recently met with an accident due to which I have fractured my right leg and the doctor has asked me not to climb stairs for at least 1 month. Since my class is on the third floor so it wouldn't be possible for me to climb stairs daily. I don't want to leave my classes since final exams are approaching.
Hope that you will understand my problem and permit me to use the lift.
Yours faithfully
ABC
