Respected Sir,
I am an old user of your bank and being a part of this is really a special honour for me. Sir, I have an account in your bank in the name of me and my father but now sadly he had expired and I don't have any desire to have a joint account. So, I have the death certificate of my father.I just want you to make an individual account for me with my name.

There is one more issue that my previous ATM card have been disappeared. I have abolished that card and now I want a new ATM card with your bank.

I am kindly requesting you to please make my individual account and also issue the new ATM card both in my name I will be always grateful to your dead.
