Sub : Regarding the issue of New Library Card

Sir/Mam,

               With due respect i am the student of your school and  i wish to say you that yesterday i have lost my library card in the school. When i reached home then only i get to know that, and i also find my library card in my home also but it was not there. Perhaps i lost it on my way back home or some one has removed it from my pocket. Anyway i have share about this to my class teacher.

Atlast, i want to say you that please issue me a duplicate or new library card and also ensure that no book is issued on that card in future. Or you can help me in finding my old card. If is it found then that's good and if is it not then please try to issue me a new library card within 2 - 3 days because i have to issue some new books from the library.
