There r a few grammatical as well as technical errors in ur application...i ll list them one by one...

Subject- ur subject should be "REQUESTING A NEW LIBRARY CARD DUE TO MISPLACEMENT" bcoz a subject is not a sentence...it is a type of phrase telling the jist of the application...

Then in ur content....i will list the mistakes.....actually it is difficult to list all the mistakes...so i will write n gv u the content...here it is...
"I Mihir Sagar of class 8th C is writing this letter to you regarding the misplacement of my library issue card. Unknowingly, last week my library card was misplaced somewhere in the school and i m unable to find it. Thus, i request you to issue another library card and assure to you that i will be responsible in taking good care of it this time.
