Subject: Application for Duplicate ID Card.

With deep regret I would like to inform you that I have misplaced my ID card. We have just shifted from our old house to a new one and in the process many of our belongings cannot be traced.  

The mid-year exams are almost round the corner and I cannot spend much time looking for my ID card. I would remain forever grateful to you if you could order the office staff to issue me a duplicate ID card.

I once again apologise to be a botheration to you. I hope you understand my plight and look over the inconvenience that I have caused to you. I will be forever indebted.
