Since you did not give your grade or age, I don’t know if you need your parents to assist you with your letter requesting a leave of four days. Your teacher will want to know the reason for the leave, the days you are requesting to be excused, what you will be doing (special event, family need, etc.), how you plan to make up for missed classes and how you can be contacted during your leave (cell phone, email). I would also ask for a list of assignments for the time you will be gone so that you can stay up-to-date.
