The form contains any of the information such as name, DOB, address, email, etc. And the form only uses to fill the above information by the user and after saving this information it will save this. The proper format of the form will be described in a manner and consist:-

• the same language of all wordings in it,  
• no extra gap between line,
• sufficient space for user entries, etc.
