First of all in the right hand side your info.. that is address,date and leave one line in the left side your school address and agin leave one line and in the middle write subject as Granting a request for one week leave.

Than start with respected sir/madam,

Myself Sanjay, from std,div,roll no of your school needs a permission for one week leave on behalf of a genuine reason that is my sisters wedding.

Its my humble request towards you that you grant me the permission fir one week leave. i promise to complete my school work on time and wont disappoint you regarding notes and studies as well.
