Essentially what the speaker is trying to say is that he/she hasn’t seen a dog in the flesh (in reality/a real dog) in 15 years except the dogs that the speaker has seen on the screen (in movies).
Another way of looking at it is, “I haven’t seen a dog since 15 years, but this is excluding the dogs that I have seen in movies.”
