Passive voice rule -

Object + was/were + verb (3rd form) + by + subject

So, the required voice change be -

This is a past Simple or indefinite tense
whose active rule = S +V's2nd form +O
and its passive rule = was/were + V's 3rd form

So now change in voice
Your father was annoyed by your voice.

we use was rather than were because this sentence is singular......
