Philip Crosby who introduced the concept of Zero Defect thinking in his book  "Quality is free" mentions

Zero defect planning requires commitment at every level of management whether it be top, middle or lower

So if there is no commitment from top level, it can fizzle up.

Zero defect planning requires total commitment of top level management.
It is adopted to prevent the maximum number of defects in the items produced. The plan is totally given by the top level management. The middle level management execute the plan by making the bottom level management to work.
