Yes, the atoms in a molecule are always arranged in a regular pattern . If their pattern is changed then the same molecule is impossible to be made. So, even if one single atom is added or if there is a change in the composition ( arrangement of the atoms ) of the molecule we may get another element or compound. Thus, the arrangement of atoms in a molecule is very important but if changed then the substance to be formed also changes.

No not always only in case of crystalline solid with long range ordered structure without any defects have regular pattern.
