Due to the pressure on the milk, the milk comes out at an increased speed (by Bernoulli's principle). The reason why the milk stops and starts is also due to this increased pressure; as the milk carton (think of it as a system) seeks to equalize pressure with the outside environment so it lets in air through the only hole in the carton. Obviously this stops or partially stops milk from flowing while the pressure is being equalized, causing the milk to "spurt". This in combination with the previous factor (increased speed due to pressure) into why the milk comes out irregular.
