Ions from sodium chloride – Na+(aq) and Cl-
Ions from water – H+ (aq) and OH-(aq)
At cathode -
2H+(aq) + 2e- ->H2(g)

Na+ ions remain in solution. 

At anode -

4OH-(aq) ->2H2O(l) + O2(g) + 4e-
