In this poem the poet draws a similarity with the palm of the hand and an old, wrinkled, map with lots of creases on it.
Then palm of the hand is like the map of the world which has been erased over and over again to draw rivers and landmarks.

This poem begins by a map of the world that is picked from a drawer. This map of the world is folded into eighths and when one takes a look at it each country bears a wrong name. ... This map has rivers coming from the heart of the country with its brother and here personification is used.
