honourly and humbly i beg to state i m xyz of class xyz . i hav written you this application to request to grant me a sick leave for 5 to 6 days . i m suffering from thypoid or (you can mention your own problem here) . doctor has advised me complete bed rest . i m not in a situation to come to the school my body has turned pale due to weakness and there is too much pain in my body due to weakness . my parents are also not allowing me to come to the school after taking a intervals of 5 to 6 days hoping that i will must be fine . hoping that you will understand my problem . for this kind consideration i shall be thankful to you forever
