Sub: Issuance of duplicate library card
Respected Madam:
I wish to state that I lost my original library card in the school premises last week. I tried to search it; I wrote a notice for the same hoping I would get it back but to no avail. My studies have been suffering as I have not been able to have any book issued without my library card. Kindly issue me a duplicate library card. I shall be grateful. Thanking you.

Subject: Regarding the issue of Library Card
Sir,
It is to bring to your notice that I have lost my original id card 2 days back. Without the library card I cannot get books from the library. Sir I am an obedient student of my class and I really need a duplicate library card as exams will be approaching soon.
I have also taken permission from the Principal and his letter is also attached with the application. Please look into the matter and issue a duplicate library card as soon as possible.
