Everyone gets sick and misses a day or two of work from time to time, and it’s important to let your employer know as soon as possible. When you find yourself in a situation in which you’ll need to take an extended medical absence from work then it is time to submit a medical leave letter to inform your employer about your situation.
Talking With Your Supervisor
It’s best to give your supervisor a heads up before formally submitting your medical leave letter. Let your boss know you would like to talk, so you can discuss your leave in private. Your supervisor and human resources department can help you with the process, as every company has their own medical leave policy. Of course this is all contingent on you knowing in advance that you will be taking a medical leave, which you may not. In case of a sudden illness or medical situation, make it a priority to call and let your boss know you will not be in. Then you can begin your formal medical leave letter. Whether it’s required or not, putting your request in writing is a good way to protect yourself, by creating a record of what transpired.
Writing Your Letter
Follow the business letter format. Briefly explain your medical situation, mention that you are providing documentation from your physician, and how long you will be away from work if you know how long it will be. It’s not necessary for you to go into the details of your condition and symptoms. “Write your letter as professionally as you would any business correspondence. Discuss how you plan to work from home, delegate assignments to colleagues, or catch up on your projects once you return,” advises Rachel Gallagher, writer at Paper Fellows. Write clearly and concisely. Don’t forget to thank your supervisor for their attention to this matter. Since you’re most likely submitting this letter in email form, write in the subject line your name and your reason for emailing.

Sample Letter
Dear Ms. Smith,
I am writing to inform you that I will be taking a sick leave because of a serious infection in my throat. I will be absent from work until October 14.
I have attached a note from my doctor to confirm that it is necessary for my health and the health of my coworkers for me to take a medical leave. I apologize for any inconvenience caused by my absence.
I do believe I will be able to complete some work and check my email from home when possible. When I return I will do my best to catch up on the projects begun before my illness.
Please let me know if you need further documentation from my doctor. I will inform you if it becomes necessary for me to take extra time away from the office. Feel free to contact me if you have any questions or need clarification.
Best Regards,
Thomas Jones
