Actually, Blood clot is formed due to the presence of a cell called Platelets. When we have an injury or wound then of course, we have blood loss from our body. So to prevent blood loss from our body, these cells help and forms a layer on that place where we have injury.
Actually, Vitamin K also helps in blood clotting.

The clotting process consists of a cascade theory. The cascade theory contains both intrinsic factors and extrinsic factors.  

The intrinsic factor includes the factors or chemicals secreted from platelets. They include prothrombin, which is a protein formed in liver. 

Prothrombin, in presence of extrinsic factor thromboplastin changes to thrombin. This helps in clotting.  

The clot blocks the wound, thereby decreases and ultimately stops bleeding.
