1) Given sentence is in passive voice. Here, the subject is ommited so while converting to active, we will add a subject which best suits on the sentence.

My new mobile has been stolen (by someone)

=> Active = Someone has stolen my new mobile phone.


2) Given sentence is interrogative past indefinite in active voice. Its passive form will be

Was the story written by Priya?

3) Given sentence is active voice present perfect tense. Its passive form :-

The prizes have been won by the girl
