I think the two boys looked after Lucia willingly for two sets of reasons.

first reason is that the two boys do their work comfortably and they have No complaints of their work they give their selfless action and respect to their cause.

the second reason is that they are regularly went to the hospital and look after her medicines and behaved to her very kindly and soft and they did not give any complaints to her

Of Course, the two brothers look after their sister lucia willingly. They have devoted their life for lucia. They were doing every possible thing which will give them money and buy that money, they will able to get a proper treatment of lucia. Moreover they daily go to see their sister and look after of everything.
