Lipase is an enzyme in the body that causes fat (lipid) break down. When insulin is present in the body, it suppresses lipase. In type one diabetes, there is no insulin, so the lipase causes fat breakdown which then causes ketones in the blood and urine. Because there is no insulin, the sugar levels are high in the blood and urine. In starvation, when carbohydrates are NOT eaten ( carbs stimulate release of insulin) the lipase causes fat breakdown so there are ketones in the urine. But, there is no sugar in the blood or the urine because of starvation. In the Atkins diet, a dipstick is used to check the urine and if it turns purple, that means there are Ketones present and that you have been good and have not eaten any carbohydrates.
