Answer:

Doctor

I have been both. Military doctors have a practice much like their civilian counterparts. We see people for appointments and do surgeries/procedures.

Millitary

We attend administrative meetings but moreso in the military. The main difference is that military doctors have to be ready to “deploy” (i.e. be sent away to a combat or supportive role in a conflict somewhere in the world). There is a great deal of training that happens with this and that can be occasional for a stateside medical center physician but can be as much as weekly if deployed in an area more prone to conflict (i.e.Korea, Middle East, etc). In general, military doctors spend too much time in their military role to keep a busy, satisfying practice. We mostly leave the service because of this.
