I think the two boys looked after their sister Lucia willingly because, no one was forcing them to work so hard to earn money.
They sacrificed their own needs to save money for operation of the their sister Lucia, who was suffering from Tuberculosis of Spine.
They tried to get best treatment out of Love, Care & Devotion for their sister.
