We can detect the defect in a metal block by following way
If there is any defect in metallic block like it breaked from centre ,there is any gap inside the block so when we pass ultrasound from it is reflect back which is observed by ultrasound detector ,and if it don't reflect this means metal block is correct there is nothing defect in it.
