There are many methods

Find the average of your two fractions, then add a very tiny irrational number to it.

The average of 13 and 12 is 512

The square root of 0.00000002 is irrational, as is e divided by a million, as is π divided by one zillion.

512+e1000000 is irrational

Find a common multiple of your two denominators that is NOT the least common multiple. What about 360? Convert both fractions to that new denominator. Pick ANY rational number between those two fractions, then add a small irrational number to the numerator. (It can be greater than 1 as long as you make sure that it does not cause your number to get too big.

What rational numbers are between 120360and180360 ? How about 127360 ?

127+e360 is an irrational number between your two numbers.

Square your two numbers. Find a rational number between those two squares that is not, itself, a perfect square. The square root of that number will be an irrational number between your two numbers.

The squares of your two numbers are 19and14

Find a rational number between 40360and90360, such as 51360

51360−−−√=51√610√=510√60 is an irrational number between your two numbers.

There are many other methods you could use.

How many different irrational numbers can you find between your two numbers? (There are an infinite number of answers.)
