Formal Charge = (Number of valence electrons) - (Non-bonding electrons) - (12 Bonding electrons)
Atomic number of oxygen = 8
Electronic configuration = 1s2 2s2 2p4
Valence electrons on oxygen atom = 6 
Non bonding electrons = Number of electrons present in lone pair
Bonding electrons = Number of bonds formed

For first oxygen atom (leftmost)
Formal Charge = 6 - 4 - 4/2 = 0

For second oxygen atom (middle)
Formal Charge = 6 - 2 - 6/2 = +1

For third oxygen atom (rightmost)
Formal Charge = 6 - 6 - 2/2 = -1
