You see there are some rules to be followed here.

ACTIVE VOICE----                        PASSIVE VOICE
1) Simple present    -------------> am/is/are + Verb in 3rd form
2) Present continuous -----------> am/is/are + being + V3
3) Present Perfect    --------------> has/have + been + V3
4) Present perfect continuous --->  x ( remains the same)
5) Simple past        -----------------> was/were + V3
6) Past continuous -----------------> was/were + being + V3
7) Past perfect    --------------------> had been + V3
8) Past perfect continuous --------> x (remains the same)
