I think anything new is creative thinking. I have always tried to think "out of the box".
An example of this occurred when I was a first year resident. We had a 16 y/o patient under general anesthesia with an undiagnosed, extremely large hydrocephalus. The attempted breech delivery had been going on for two hours. The baby was dead by then.
The chief attending physician was attempting to deliver the baby by any means possible. The head was the size of a huge watermelon, and consequently they felt a Cesarian Section was not indicated.
It seemed a simple solution to me at the time to stick a needle into the abdomen, drain the fluid out of the baby's head, and then deliver the baby. This was unheard of at the time, and being a first year resident, I was ignored.
It ended up with the head being destroyed by a huge instrument, a ruptured uterus, a dead baby, and a hysterectomy - in a 16 year old.
Today amniocentesis (inserting a needle into the abdomen) is an extremely common procedure.
