Answer: A black hole forms when a dying supermassive star over 3 stellar masses collapses over its own mass. After it explodes in a type II Supernova, it forms a black hole.
Don't be confused with neutron star. A neutron star forms in the same process, but with a massive star under 3 stellar masses.
